Service Description

Our learning disability service is dedicated to providing high-quality care and support to individuals with specific diagnoses of learning disability and autism. We understand the unique challenges that come with these conditions and strive to create a safe and nurturing environment for our clients. Our committed team experienced staff use the approaches underpinned by the REACH standards in meeting the daily needs of people in our care.

Service Goals

The main goal of this service is to facilitate an environment that our supported individuals can call home. The focus of each provision is to offer services that enhance independent living skills within daily activities, programs and outcomes agreed prior to commencement or as part of individualised support packages. Within this community, individuals are supported in a home environment which they occupy under their own individual tenancies. Each person is allocated a dedicated Key Worker who ensures that individuals always have their voices heard based on their choice, respect and individuality. At the heart of this provision is the facilitation and encouragement of family inclusion in individual support. We consider this a crucial part of individuality and uniqueness which defines our people as equal members of their local communities.

Care Approach

Our learning disability service is focused on using the REACH standards in aligning support provision with individual needs. Whether you are someone who is interested in arts or gardening, we ensure that we fully capture your interests in our assessments before you move in with us. We then put a personalised support plan in place as a guide for meeting your needs in a person-centred way.

Eligibility

Our learning disability services are available to any individual aged 17 or above with a learning disability, autism or mental health difficulty who requires housing and support within either a single or group living environment. In specific cases where we have the available resources, we may also be able to provide support within your privately owned or rented property. In this case, a minimum of 8 hours of clustered support may be required per day, to ensure we can offer you a quality service.
