Service Description

We are dedicated to providing high-quality care and support to individuals struggling with mental health conditions. Our team of experienced professionals is committed to using our in-house recovery model in supporting people discharged from hospitals to achieve their goals and improve their overall well-being in a quiet and welcoming environment. 

Service Goals

The purpose of this service is to prevent unnecessary hospital admissions and bed-blocking within mainstream local NHS installations. This is achieved by facilitating professional input in treatment and recovery in a beautiful individualised environment. At our mental health supported living location, we work closely with mental health professionals to ensure continued recovery for our individuals to provide a safe route to complete rehabilitation, health and functioning.

Care Approach

Referrals to this service are usually made via email to our placement team. In cases where a referral has been received by telephone call, this must also be followed up in writing with the complete profile of the individual. This will usually include their current risk assessments and support plans. We also emphasise receiving full information on the individual's background, preferences and experiences, after which we will then arrange to conduct our full initial assessments to ascertain the associated risks and needs of the individual and to establish any areas where we require further information.

Eligibility

Our mental health supported living services are available to any individual aged 17 or above with a mental health illness diagnosis who requires housing and support with their recovery process.
